New Client Forms
There are 2 forms that need to be filled out for our first appointment which include 1) new client paperwork and 2) adult intake or child intake form. Please note that forms will need to be submitted electronically by fax 760-334-8767 or i will send you a secure email that you can respond to so they are sent in a hipaa compliant format. I have also included a copy of my HIPAA notice of privacy practices for your reference.
